I was told the form module with DNN isn't very reliable. If this is true, where can I find a form module that works and is reasonably priced? I need one that allow me to create a form where you enter name, address, city, etc and also have the ability to ask questions and let viewers click a yes/no button. I need this info to be e-mailed back to me. Any suggestions?

Take a look at Form Master 2008 and Dynamic Forms. You'll find both at Snowcovered. There are others, but those are the modules that I have used. If others have recommendations, please post them here.

Don't discount the Form and List Module for relative simple forms.

Sure.

Is best to have a development site set up. You can easily install and test modules without impacting a live site.
