One on my clients is requesting that I change their registration screen to look like the following:

I tried going through the video tutorial on this site, but that still didn't help.  The problem seems to be getting all the fields in the top, without a heading and in a double column layout.

Any help would be greatly appreciated.

That double column layout is the layout of earlier versions of DotNetNuke, the latest version of DotNetNuke uses a single column layout. If you wish to create a double column layout, I think the only way to do this will be to create your own custom registration module.

The title comes from the category that each field is placed under, if you put them all in the same category they will all be grouped under one title. I think this is covered in videos 2 and 3 of: How to Manage Profile Properties in User accounts (Creating a custom registration page)
