Try using the Newsletter to sent an email to an address outside of your domain. This will demonstrate that you can do that.

When I send an email from the Newsletter module I get a SMPT configuration Test email in return but not the email. Any ideas?

You should not get the SMTP Configuration Test message, but you (as site administrator) should get a report on the mail delivery.  If you choose the synchronous mail option, you might see some additional details to help debug.

Without knowing more, I would guess that the SMTP server that you are using is refusing to send the mail.  This might be an anti-spam setting.  Check with whoever administers the SMTP server for help.  There are SMTP log files that should show more information.

Well after playing with settings I figured it out.

I was using my admins email address in the FROM section. This email is my regular email and not associated with the domain where dnn is hosted.

I changed the FROM line to an email address associated with the hosting domain and all works fine now.

This is confusing in the DNN set up and not explained that it will fail without the FROM section set properly. I set up an email newsletter@dnndomain.com and a few others just to make things look authentic and problem solved.

That is not a DotNetNuke issue, but is related to the setup of your SMTP server.  I think that your SMTP server is set so that it will not relay for sending email addresses outside of your site's domain.  This is done to prevent spammer from using your SMTP server from sending mail for them.

If you need an SMTP server set to send mail for a specific email address, this can be configured by whomever administers the SMTP server.

I went that route before this and included 'safe' domains in my hosts smtp set up and set the ones I use as safe but that did not help.

Any application built in DNN needs to have an email going functionality, for example, DNN sends an email to the user registers on the site/ portal. The other example includes, the forum module sends an email to the owner of the threads when a reply has post etc... and it is also necessary for the site host to send Newsletter email to all register user of the site/ application.
