Feedback Module-Default values

When I add the feedback module to my page it automatically has populates the email and name values. Where do I need to go to make those default values blank? Thanks

If you are logged in, those are filled in automatically. The assumption is that the feedback is coming from the logged in user. If you are not logged in, those items will be blank, so that a non-logged in user will have to fill them in.
