I want to add new page change password , which module i have to include in my page to display only reset password options ?

The View Profile module includes the functionality to change or reset the user's password. I don't know if you can set up the module to display only that functionality, but maybe.

If not, you at least know where to get some code that will accomplish it.

You can write your own using the Razor host module.
