I'm looking for a module that will provide a good survey application. My survey will ask clients to review a long array of treatment modalities for a disease. It has to have a comment section and either buttons or a slider bar. I also need the survey to automatically update pages reviewing these products. Any recommendations?

Either Form Master 2008 from Code Five Systems or Enterprise Forms from Euthonmai. Both available on Snowcovered.
Form Master is very comprehensive and will be able to do what you want. Its also available on a 15 day download demo, so you can test it out.
Enterprise Forms is very complicated and fully featured. It takes a while to learn how to use it. But once you understand how it operates you will find it very helpful.
